The purpose of this form is to transmit your self-assessement report.

Your are asked to mention values from 1 (poor) to 5 (excellent) for each competence:

• 1 - Poor / Not enough

• 2 - Weak / starts to understand

• 3 - Sufficient / passed

• 4 - Good / understood & applies

• 5 - Excellent

If you fill in the form multiple times, only the last one will be taken into account.
